it's not merely an abscess


just thought to share with u a case i've clerked in the gynaecology ward yesterday.

a 22 y/o female Muslim, enganged, presented to hospital with a swelling at the private part, associated with pain and mild fever for 6 days. on examination, it was a swollen left bartholin's cyst abscess. I&D was done. on further questioning, the girl admitted that she has active unprotected sexual intercourses with her fiance. they planned to get married in this coming December.

hm..
for non-medical readers, Bartholin's gland is situated at the female private part, which normally produce 'moisturizer'. it is normally not palpable unless it is blocked or infected. usually it is associated with sexually transmitted disease but it is not an exclusive cause.

if you were the doctor, how would u counsel her?
as a Muslim doctor, how would u tackle this problem?
as a Muslim, how would u tell the girl that her act with her fiance is a sin?

when i first see this case and i knew the story, i was like- padan muke, why u have sexual intercourse before married?
on the second thought, who am i to punish her, isn't it. what she has done is not my problem. it is the future that is matter most. what can i do as a Muslim sister to prevent her from repeating the same sin? it was a tough job. she knew that her act is a sin. 'kalau parents saya tau ni mati la saya'.. from my judgement, she is not from a very well educated background. to tell and lecture her on how her act is a sin is clearly not a good move. in fact, i had to level down to talk to her. n somehow, i managed to talk to her regarding the 'problem'. it was a very brief one but i think i did deliver my intention and she seems to accept it. Alhamdulillah.. at least i have done something good during my on-call on sunday night =)

well, that was not the first time i encounter such a social problem of pre-marital sexual intercourse. it is indeed a hard situation to face to. the problem is more than just an abscess, more than just an ectopic pregnancy. well, as a Doctor, u r not supposed to mix ur religious principle with other people's. but as a Muslim, u have the obligation for 'amal maaruf nahi mungkar'..

a right way at the right time is the solution, i think. don't u think so? =)

ohh..I&D tu = marsupialization tak?

Last year, kat HSA, aku ada tgk marsupialization for bartholin cyst.. yang aku rasa cam incision and drainage lee..

marsupialization slalu for bartholin's cyst.. yg xde abscess..
kalo da infected & ada abscess, buat I&D. rasanya la.. wutever it is, the cyst can recur.

yup, difficult.. as a Dr, ada principles & ethics yg kena follow. tp tgk la, kalo seswai ngn cara & timing, rsnya shudnt b a big problem to talk about that. need experience to handle such a situation. may Allah grant me the strength, n to all my colleagues too.. amin =)

ampun..
for bartholin's abscess tu, ye la, after drainage kena buat marsupialization jugak..
tp marsupialization tak sama dgn i&d kan.. lain sbb dia tros let the gland open up.
btw, thank sbb betulkan.
